📋 Tanggung Jawab Pekerjaan

• Develop and implement personalized care plans for patients in collaboration with healthcare professionals
• Provide emotional support and companionship to patients and their families
• Coordinate and arrange necessary medical services, such as home health aides, nurses, or medical equipment
• Educate patients and their families on managing chronic illnesses or disabilities
• Monitor patients' health status and report any changes to healthcare providers
• Assist in organizing and participating in community health programs and workshops
• Maintain accurate and confidential patient records and documentation
• Promote a positive and supportive environment for patients and their families

🎓 Kualifikasi & Syarat

• Bachelor's degree in Social Work, Nursing, or a related field
• Minimum of 2 years of experience in home care or a similar role
• Certification as a Medical Social Worker (CSW) or comparable certification
• Strong knowledge of home care best practices and regulations
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Proficiency in English and Bahasa Indonesia
• Valid driver's license and reliable transportation
